After Action Reviews should be adopted by HSE

Dear Editor, I read with interest the recent front-page report by Lloyd Mudiwa, ‘HSE set to push local adverse event reviews’ (http://bit.ly/14YLrif),  and its continuance on page 9 of the June 21 edition of Irish Medical Times. The Director of Quality and Patient Safety (HSE) Dr Philip Crowley advocates a new system of adverse event reviews in a fair and open manner. There may be a misquote in the last paragraph of the article concerning an update of the current policy, the latter of which I am ignorant.
