Patient Focus is advocating for an urgent review into how adverse events are investigated in Irish hospitals, claiming many injuries and deaths of patients were not investigated promptly, thoroughly and fairly. Commenting ahead of the publication of the report of the investigation into the tragic death of Savita Halappanavar in Galway University Hospital last October, the patient advocacy organisation said it was “strongly” of the opinion that an urgent review needs to be conducted into the manner in which deaths and injuries to patients are investigated in Irish hospitals to bring them up to an acceptable standard.…
